One recommendation must be from a licensed CRNA that the applicant has shadowed; one recommendation must be from the immediate supervisor, and one recommendation must be from a current RN/APRN co-worker. Recommendations from friends or family members are not accepted. Only NursingCAS recommendation/evaluation forms will be accepted. Written letters of recommendation are not accepted.

Calculation of Science GPA is all sciences courses taken.
